Question:
When is endarterectomy separately reportable in the setting of an artery repair (direct and/or EVAR)? Are there any guidelines/resources I can provide to my provider regarding this?

Answer:
An endarterectomy in the setting of an aneurysm repair or a bypass graft or other primary procedure is considered bundled if the same vessel. For example, if you do a common femoral endarterectomy and then anastomose a fem-pop graft to that endarterectomized vessel, you only code the fem-pop and not the endarterectomy. This is clearly stated in the NCCI Manual, Chapter V, in the cardiovascular system section. If an endarterectomy in a different vessel like the contralateral limb is performed, it would be reported.
